The information collected (name, student’s name, email address, phone number and any narrative info you opt to share) will be used only internally to direct the proper staff person to schedule a meeting (call/session) with you.
When you complete the New Speller Intake packet:
After the preliminary direct contact with a staff member, in order to establish a client account with Transcending Apraxia we use FormDr. Com, a HIPAA compliant third-party site designed to safely collect demographic and medical history. FormDr collects personally identifiable information about you and is authorized to maintain a record of the information related to your enrollment.
None of your identifiable information will ever be disclosed to other agencies, providers or practitioners without your prior written request and consent. To request that your child’s records be shared with another provider please request and complete TA’s Authorization to Release Records. Upon receipt of the signed document, the specific records requested will be released.
Security
The security of your Personal Information is important to us, but remember that no method of transmission over the Internet, or method of electronic storage is 100% secure. While we strive to use commercially acceptable means to protect your Personal Information, we cannot guarantee its absolute security. Consequently, use of TA and its resources constitutes an acknowledgement by the user that they release TA from all liability resulting in any damages relating to a malicious attack on the site or any external interference outside of TA’s knowledge or control that causes damage to the user.
